Registered Nurse - MHICM (Mental Health Intensive Care Management)
The incumbent is responsible for coordinating the day-to-day provision of mental care and case management services to assigned Veterans, assisting with follow up, alliance building, and liaison with other disciplines, caregivers, family members and community resources. The incumbent works with members of an interdisciplinary screening team in responding to consults for MHICM services. This requires familiarity with the care delivery process and resources. The incumbent assists participants' families and clinical team members in coordinating care in CAVHS and the community and resource referrals if needed, when clinically indicated, and works in unison with the Interdisciplinary Treatment teams in promoting a Patient-Centered culture; assuring that quality health-care services are provided to the Veterans promptly, safely, courteously and compassionately as well as their significant others and/or major stakeholders are appropriately involved. The incumbent provides patient, family, and/or caregiver education and support, as well as actively promotes patient self-management and personal health responsibility. The incumbent is responsible to communicate with all members of the healthcare team on behalf of the patient and has direct involvement with the development and enhancement of processes with the aim to improve the clinical experience for Veterans, families and clinical staff members. The incumbent communicates effectively both orally and in writing with professional, administrative and technical staff at all levels. The incumbent functions as liaison between MHS internal and external resources, including community agencies, guardians, conservators and providers concerning appropriate notifications. The incumbent collaborates and shares information with medical and/or clinical treatment staff members regarding potential improvement in patient care delivery through comparisons to community standards. The incumbent demonstrates effective communication skills and behavior that promotes professional cooperation and team building with internal and external customers. The incumbent shares information to assure continuity and safety in area of assignment and collaborates with co-workers in completing workload. The incumbent generates Treatment Plans and participates on teams, contributes to team conferences and is actively involved in planning for continuity of Veteran care. The incumbent consults with other healthcare providers to formulate a collaborative plan of care. The incumbent provides education related to the provision of mental health care. The incumbent performs work related to the review, monitoring, analysis, planning and assessment of processes utilized by the MHICM program. The incumbent performs other related duties as assigned. This position requires visual acuity (including depth perception), keen hearing (aide permitted), clear distinctive speech, and manual dexterity in both hands. This position requires potentially long periods of continued walking (2-4 hours), standing (2-6 hours), stooping, sitting, bending, pulling (1-2 hours), pushing (1-2 hours), lifting (>50 pounds), and carrying (>50 pounds). Transferring patients and objects may be required. Mechanical inpatient lifts are provided. The incumbent may be exposed to infected patients and contaminated materials and may be required to don protective clothing in isolation situations, operative/invasive procedures, or home settings. The incumbent may be exposed to patients who are combative secondary to delirium, dementia, or psychiatric disorders. Ability for rapid mental and muscular coordination simultaneously. The incumbent must be a mature, flexible, sensible individual capable of working effectively in stressful situations, able to shift priorities based on patient needs. Must complete annual Employee Health requirements, such as annual TB screening or testing, as a condition of employment. Must be able to work closely with others, as well as protracted or irregular hours of work. Must be compliance with the use of ergonomic exercises and employee safety requirements.
